The Horror of Invisible Pain: Fibromyalgia and Japan’s Designated Intractable Diseases

In the film The Trembling Tongue, one of the most unforgettable images is a young girl whose central nervous system has been attacked by tetanus bacteria: she screams, convulses, and even bites her own tongue. The fear of something invisible invading the nervous system is almost impossible to put into words.

But infections are not the only illnesses that can assault the nerves or the organs of the body. There are many diseases that do this, and one of the better-known examples is Ménière’s disease, an illness whose cause is still said to be unclear.

With Ménière’s disease, a person may suddenly experience vertigo so intense that the scenery seems to spin in circles. Medication can control it to some degree, but neither the patient nor the people around them know when the next attack will come.

Lying in bed, the ceiling seems to warp and rotate. It can feel like being on a ferry, and in severe cases that sensation can last for days.

Ménière’s disease is not usually associated with pain. But among illnesses that affect the nervous system, some are defined above all by severe pain. One of them is fibromyalgia.

What Is Fibromyalgia?

According to Medical Note, fibromyalgia is a disease that causes chronic, severe pain throughout the body, including the joints, muscles, and tendons. Because the pain can be intense, it may also lead to insomnia, stress, and depression. As of March 2018, no treatment had been found that was guaranteed to be effective, so care mainly consists of symptomatic treatment, including medication and non-drug therapies.

Because medical tests may not show clear abnormalities, the key symptom is the patient’s subjective experience of pain. That also means understanding from the people around the patient is essential.

According to Japan’s Rheumatism Information Center, fibromyalgia affects about 1.7% of the population, or roughly 2 million people. Rheumatoid arthritis affects about 0.7% of the population, meaning fibromyalgia has more patients than rheumatoid arthritis. Yet it remains far less widely known.

Fibromyalgia is characterized by pain that runs through many parts of the body, even though tests may show no abnormality. Many patients experience pain so severe that they can no longer live their daily lives as before. But the cause is unknown.

Because the cause is unknown, there is no fundamental cure. Treatment focuses mainly on coping with the pain.

The Rheumatism Information Center explains fibromyalgia as a possible malfunction in the brain’s neurotransmission system that carries pain signals. In other words, the person feels pain or stimulation even when there is no corresponding injury or stimulus. It is like the accelerator for pain has been pressed down and the brakes no longer work—yet no visible abnormality can be found.

It is a condition like a bug in the computer of the brain, where pain can no longer be controlled. But no one knows exactly where the cause lies.

When Pain Becomes a Trigger for Depression

One person who developed fibromyalgia described their pain in terrifying terms:

“The trigeminal nerve feels wrong first. It feels like an arrow is stuck into my temple. Like a bed of nails on the top of my head, with spasms and numbness through my lower jaw. Ice picks stabbing into my eyeballs and optic nerves. A hammer constantly pounding the back of my head. My trapezius muscles are always tight, like I’m carrying a stone statue on my back.

“From my upper arms to my fingertips, it feels like hundreds of centipedes are crawling through my flesh. From my back to my lower back, there is a dull pain I can’t properly describe. If I had to compare it to something, it feels like being hit by a car, endlessly.

“My legs are the same as my arms, with hundreds of centipedes crawling around. At night, I can’t sleep because of the pain. Even the weight of my own body hurts, and turning over in bed is painful enough to wake me up.

“During the day, just sunlight entering my eyes becomes a storm of ice picks. In the office, fluorescent lights feel like repeated ice-pick blows to the eyeballs. Even when I’m exhausted, the pain will not let me sleep. It continues 24 hours a day, 365 days a year.

“In the end, every single movement turns into pain. Medicines such as Loxonin, Voltaren, and Lyrica don’t work. Heat therapy finally eases it a little, but once the warmth fades, the pain returns.

“And then your mind starts to break. You begin to think, ‘I can’t take this anymore. Just kill me.’ Think of it as being tortured without end. Torture every time you move. Torture that doesn’t even allow sleep. You can’t swallow food.

“Many people die by suicide. If this torture happened every day, of course people would think that way.

“When you go to the hospital, many doctors don’t even know the name fibromyalgia. Painkillers, psychiatric drugs—huge amounts of medication, just to knock out your consciousness. Only symptomatic treatment. No treatment for the root cause.

“Then the body weakens. Up to stage 3, many people can manage daily life somehow while suppressing symptoms with medicine. Stage 4 is described as pain many times stronger than childbirth, and stage 5 as pain stronger than that of a terminal cancer patient.

“Even so, morphine and similar drugs cannot be used. In many cases, American medicine is said to have better recognition and treatment options, so people with money sometimes go to the United States. Recently, marijuana treatment became possible in Thailand, and fibromyalgia patients from around the world have been going there.”

This is one patient’s account, but it conveys the horror of severe pain that appears without visible abnormality. Because fibromyalgia has no established cure, treatment is centered on managing symptoms.

When intense pain continues all day, every day, with no cure in sight, it is not unusual for patients to develop depression or other mental health problems. If a person must endure these symptoms day after day, it is not hard to understand why their mind would suffer as well.

Some sources say Kampo, or traditional Japanese herbal medicine, may help ease pain in some cases. But at present, it is not a fundamental solution.

The Fear of a “Bug” in the Brain

The human brain controls many things, including pain and memory. Among illnesses said to have unknown causes, some destroy brain cells, while others seem to create pain through a malfunction in the brain itself.

In The Trembling Tongue, tetanus bacteria attack the central nervous system, and the person loses control of their own body and behavior. Fibromyalgia is different: no obvious abnormality may appear, yet severe pain spreads through the entire body.

Tetanus can sometimes be treated, but for fibromyalgia, there is currently no cure. Once it develops, many patients have no choice but to take medicine and endure the pain.

Another disease that affects nerve cells in the cerebellum and spinal cord is spinocerebellar degeneration, made widely known in Japan by One Liter of Tears. Like fibromyalgia, it can be especially cruel because the person’s consciousness remains clear. The suffering after onset may be far beyond what outsiders can imagine.

These are illnesses described as having unknown causes. That means any one of us could, one day, suddenly be attacked by such a disease.

Fibromyalgia Is Not a Designated Intractable Disease in Japan

Fibromyalgia causes unexplained pain to run throughout the body. Even so, in Japan it is not classified as a “designated intractable disease.”

If a disease is designated, patients may become eligible for medical expense subsidies. In 2018, the number of designated diseases had expanded from 56 to 331. Fibromyalgia, however, was not included.

When a disease is not designated, it becomes harder for patients to receive public support, leaving them in an even more difficult position.

Why would an illness with an unknown cause not be designated as an intractable disease? One reason is the “0.1%” requirement.

Under Japan’s Act on Medical Care for Patients with Intractable Diseases, one requirement is that the number of patients in Japan must not reach a certain level—about 0.1% of the population. Another requirement is the existence of objective diagnostic criteria, or something equivalent.

Fibromyalgia is not designated because there are too many patients.

That fact is bitterly ironic. Because the patient population is large, the disease does not qualify for the designation. Some patients may be able to receive disability pension benefits, but it is hard to say that public care is sufficient.

If a disease with many patients were designated as intractable, it would require a greater amount of public funding. Japan has welfare systems that provide support for children and older adults, but at the same time, we may need to pay closer attention to the many people suffering from illnesses that are not designated and therefore receive less support.

Conclusion

Diseases that affect the central nervous system or the brain are deeply frightening. Many have no cure, and if they develop, patients must often rely on symptomatic treatment.

Fibromyalgia is an intractable illness involving severe, invisible pain, yet it is not classified as a designated intractable disease in Japan. Because the pain cannot be seen, it is also difficult for others to understand.

Perhaps our bodies are ruled by things we cannot control: the central nervous system, the brain, and even our genes.

映画「震える舌」 では、破傷風菌に中枢神経を犯された少女が舌を噛み切ったり絶叫したりするシーンが印象的でした。中枢神経を目に見えないものに犯されていく恐怖は筆舌に尽くしがたいものですが、感染症意外でも神経や身体の器官を犯す病はたくさんあります。例えば、メニエール病などは原因は不明といわれている病気のひとつでよく聞く病のひとつです。メニエールになると、ある日突然、景色がグルグルと回るような目眩が起こります。薬である程度はコントロールできますが、いつめまいが来るかは本人にも周りにもわかりません。

寝ていると天井がぐにゃりと動いて回る…まるでフェリーに乗っているような感じが長い場合は、何日も続きます。メニエール病には痛みはないのですが、神経を犯す病の中にはとにかく痛みが強い病もあります。そのひとつが”線維筋痛症”という病気です。線維筋痛症とは。線維筋痛症 『線維筋痛症とは、関節や筋肉、腱など全身にわたり慢性的な激しい痛みが生じる病気です。強い痛みがあるため、不眠やストレス、抑うつ状態を引き起こすこともあります。現時点では確実に効果のある治療法がみつかっておらず、薬物療法や非薬物療法を含めた対症療法が主体になります(2018年3月時点)。

検査所見で異常を認めることができないため、「痛み」という主観的な自覚症状が鍵となる病気であり、周囲の理解も必要とされます。』。引用 メディカルノート線維筋痛症 。線維筋痛症の患者数は、 リウマチ情報センターによると人口の1.7%の約200万人 です。リウマチは、人口の0.7%ですから、リウマチより線維筋痛症の方が患者数が多いんです。なのにあまり知られていない病気でもあります。。線維筋痛症は、身体の至るとこに痛みが走る病気で検査をしても異常が見られないのが特徴です。

痛みが強く日常生活が送れなくなる患者さんも多いのですが、原因は不明…。原因が解らないので、治療法もなく痛みに対する対処療法がメインです。リウマチ情報センターのホームページでは、線維筋痛症は痛みを伝える脳の神経伝達機能に何らかの不具合が生じたもので、痛みも刺激も現実にはないのにそれを感じてしまうとされいます。痛みのアクセルが踏まれた状態でそのブレーキが気かなくなる…だけど、異常はどこにもない…。脳というコンピューターのバグで痛みのコントロールができなくなった状態だけど、どこに原因があるのかは解らないのです。

参考資料: リウマチ情報センター 。線維筋痛症はうつなどの病気の引き金になることも…。。。線維筋痛症を発症した方は、自分の痛みと症状をこのように表現されています。『三叉神経がまずおかしい。。コメカミに矢が刺さる。。頭頂部に剣山、下顎全体に痙攣や痺れ 。眼球、視神経にアイスピック滅多刺し 。後頭部ハンマーで連打され続ける 。僧帽筋は常にパンパン、石像背負わされてる 。二の腕から指先にかけては百足が何百匹も肉の中を這ってる 。背中から腰にかけては言い表せない鈍痛。

強いて言えば車に轢かれたような感じが延々と続く。。脚に関しても腕と同じで百足が数百匹這いずり回ってる。。夜は痛みで眠れず、自重の重みが痛みとなり寝返りをうつだけで目が覚めるほど痛む。。昼は日の光が目に入るだけでアイスピックの嵐。オフィス内においては蛍光灯の光が眼球アイスピック連打。。疲れ切っても、眠ることすら許されない痛みが24時間365日。。最終的には一挙手一投足が全て痛みに変わり、ロキソニンやボルタレン、リリカなどでは効かない。。温感療法を用いてようやく少し緩和する程度。

。しかし冷めるとまた痛みの連続。そして精神がおかしくなってくる。もう嫌だ殺してくれ、となる。拷問を受け続けていると捉えてくれれば良い。動く度に拷問。寝る間も許されない。食事も喉を通らない。。自殺者多数。そりゃこんな拷問毎日なら当然そう思う。。病院にいけば、線維筋痛症の病名すら知らない医師が多数。。痛み止め、向精神薬など、大量薬漬け、意識を飛ばして、対処療法のみ。根幹治療は無し。。そして身体は衰弱していき、ステージ3まではなんとか薬で抑えながら日常生活は送れる人が多いが、ステージ4は常に出産の痛みの10倍数値、ステージ5は末期癌患者よりも強い痛みを伴う。

。尚、モルヒネなどは利用できない。。アメリカ医術の方が認知度、対処共に優れていることが多く、金銭面で余裕があればアメリカに渡航する人も多い。。近々ではタイでマリファナ治療が可能になった為、世界中から線維筋痛症患者が殺到している。』 。どこにも異常はないのに激しい痛みだけがある…線維筋痛症は治療法がないこともあって対処療法がメインです。痛みが強く一日中続いて治療法もない…そんな状況に追い込まれた患者さんが心の病のうつ病を発症するケースも珍しくありません。

人間の脳は痛みや記憶など色々なものをコントロールする器官です。原因不明といわれる病の中には、脳の細胞が壊されるものや脳のバグで痛みを発症するものなどがあります。映画『震える舌』のように、中枢神経に侵す破傷風菌に感染すると人は自分の身体や行動をコントロールできなくなるわけですが、線維筋痛症のようにどこにも異常がないのに全身に強い痛みをおこす病気もあります。破傷風菌は、感知も望めますが 線維筋痛症は今のところ治療法がなく、発症すると薬を飲んで痛みに耐えるよりほかに方法はありません。

。そして、小脳や脊髄の神経細胞に異常をきたす病としては小説『一リットルの涙』で有名になった精髄小脳変性症などがあげられます。精髄小脳変性症もそして、線維筋痛症も 意識ははっきりしているため発症した場合の苦しみは想像以上に辛い ものとなります。原因不明と言われている病ですから、私たちの誰もがある日突然、病に侵される可能性もあるのです。線維筋痛症は指定難病ではない。原因不明の痛みが身体中を駆け巡る線維筋痛症ですが、指定難病になっていません。

指定難病とされると医療費助成の対象になり、平成30年には56疾患から331疾患に拡大されていますが線維筋痛症はその指定から除外されています。指定難病ではい場合、公的な補助を受けにくくなり患者さんはよりつらい立場に立たされることになります。原因不明の病がなぜ難病指定されないのか…それには 「0.1%」という指定難病の要件 があるからなんです。難病の患者に対する医療等に関する法律で 「 患者数が本邦において一定の人数(人口の約0.1%程度)に達しないこと; 客観的な診断基準(またはそれに準ずるもの)」 とされているのです。

線維筋痛症は患者数が多いから指定難病になっていないんです。患者数が多いから指定難病にならない…障害年金を受給できるケースもありますが、十分な公的なケアを受けられているとは言い難いのが現状です。患者数が多い病気を指定難病にすれば、それだけ公的な費用を投じることになる…子供や高齢者への手厚社会福祉制度がある一方で、こうして 指定難病にもされずに苦しむ多くの人がいることにも私たちは目を向ける必要がある のではないでしょうか。参考資料 厚生労働省 指定難病 。
